How Much Lip Filler Should You Have?

One of the most common questions we hear at Karwal Aesthetics is deceptively simple: how much lip filler do I actually need? It's a fair question — and the answer matters more than most people realise. Too little and you won't see a difference. Too much and you risk the overfilled look you were trying to avoid in the first place.

The truth is, the right volume depends entirely on your anatomy, your goals, and how your lips respond over time. But there are some principles worth understanding before you book your appointment.

Less Goes Further Than You Think

For most people, 0.5–1ml of dermal filler is enough to create a noticeable, natural-looking improvement. That might not sound like much, but a skilled injector can use that volume to add definition to the lip border, balance asymmetry, or restore fullness that's been lost with age — all without tipping into "overdone" territory.

If you're having lip filler for the first time, starting with a conservative volume is almost always the right call. You can always add more at a later date, but correcting overfilled lips is a far more involved process.

What If You Want a More Dramatic Result?

If you have naturally thin lips or you're looking for a fuller, more sculpted look, a single session may not get you where you want to be — and that's by design. At Karwal Aesthetics, Dr Arun Karwal recommends starting with 1ml and then reassessing after six months to a year. This staged approach gives the tissue time to settle and stretch naturally, so subsequent filler sits more comfortably and lasts longer.

Rushing that process — packing in too much volume too soon — is one of the most common mistakes we see from patients who've had treatment elsewhere. It's also one of the leading causes of filler migration, where product drifts beyond the lip border and creates a blurred, shelf-like appearance above the upper lip.

The 1ml-Per-Year Rule

As a general guideline, Dr Karwal advises that no more than 1ml of filler should be placed in the lips within any 12-month period. Beyond this threshold, the risk of migration increases significantly — and it becomes much harder to maintain a natural shape.

This isn't a universal rule across the industry, but it's one we stand by. We'd rather you come back for a measured top-up than deal with the consequences of overfilling, which can include difficulty speaking, discomfort, and an unnatural appearance that's difficult to reverse without dissolving.

Why the Consultation Matters

Every set of lips is different. The volume and placement that works beautifully on one person can look completely wrong on another. That's why we dedicate extended consultation time before any treatment — it's not just about how much filler to use, but where to place it and how your lips are likely to respond.

During your consultation, Dr Karwal will assess your lip anatomy, the proportion of your lips relative to the rest of your face, and any previous filler that may still be present. If you've had filler elsewhere and aren't happy with the result, we can discuss dissolving before starting fresh — something we specialise in using ultrasound-guided techniques for precision and safety.

The Bottom Line

The best lip filler results come from restraint, not volume. A skilled practitioner who understands facial proportions and takes a gradual approach will always deliver a better outcome than one who simply injects as much as you're willing to pay for.

If you're considering lip filler — whether it's your first time or you're looking to correct previous work — book a consultation at Karwal Aesthetics to discuss what's right for you.
