What Are Nasolabial & Marionette Lines?

Nasolabial Lines

Nasolabial folds, often referred to as "smile lines" or "laugh lines," are the creases that extend from the sides of your nose down to the corners of your mouth. While they can add character when you smile, deepened nasolabial lines can make you appear older or more tired than you feel.

Marionette Lines

Marionette lines run vertically from the corners of your mouth down towards your chin, creating a downturned appearance. The name comes from the resemblance to the mouth lines on a marionette puppet. These lines can give the face a sad or stern expression, even when you're feeling happy.

Dr Arun Karwal is the founder and lead practitioner at Karwal Aesthetics.

Treating These Lines with Dermal Fillers

Dermal fillers represent the most effective non-surgical solution for nasolabial and marionette lines. At Karwal Aesthetics, we exclusively use premium hyaluronic acid-based fillers, which offer exceptional results through their unique properties.

How Hyaluronic Acid Fillers Work:

  • Volume restoration: The filler replaces lost volume deep within the skin's structure, lifting and supporting the overlying tissue from beneath

  • Crease smoothing: By filling the space beneath deep folds and creases, the filler dramatically softens their appearance, creating a smoother, more youthful contour

  • Instant transformation: You'll see visible improvement immediately following treatment, with results continuing to enhance as the product integrates with your tissue

  • Collagen stimulation: Beyond immediate volume replacement, hyaluronic acid triggers your skin's natural healing response, encouraging fresh collagen production for longer-lasting improvement

  • Natural hydration: Hyaluronic acid attracts and binds water molecules, providing deep hydration that enhances skin quality and texture

The treatment is precise, customisable, and designed to create natural-looking results that enhance your features whilst maintaining your unique facial expressions and character.

What Causes These Lines?

Several factors contribute to the development of nasolabial and marionette lines:

  • Natural ageing: As we age, our skin loses collagen and elastin, leading to reduced firmness and elasticity

  • Volume loss: Fat pads in the face gradually diminish over time, causing skin to sag

  • Repetitive facial movements: Smiling, talking, and other expressions create creases over time

  • Sun damage: UV exposure accelerates collagen breakdown

  • Lifestyle factors: Smoking, poor nutrition, and inadequate skincare can worsen these lines

  • Genetics: Some people are predisposed to developing deeper facial lines

FAQs

  • It's important to understand that whilst we can significantly soften and reduce the appearance of nasolabial lines, we would not recommend attempting to completely erase them. The result would look very unnatural and actually detract from your appearance rather than enhance it.

Why Nasolabial Lines Are Natural and Important

We often remind our clients that even children have nasolabial lines. These lines are a natural part of facial anatomy and serve important functions:

    • Facial expression: Nasolabial folds are essential for natural facial movements and expressions. They allow your face to move naturally when you smile, talk, and show emotion

    • Facial structure: These lines define the contours of your face and contribute to your unique facial character. They help distinguish different areas of the face and provide natural definition

    • Natural appearance: Completely smooth nasolabial areas would create an artificial, "overdone" look that doesn't reflect natural human anatomy. The goal of aesthetic treatment is enhancement, not erasure

    • Age-appropriate beauty: Some degree of these lines is normal and expected at every age. Our aim is to soften deepened lines that have formed due to volume loss and ageing, not to create an unnaturally smooth appearance

At Karwal Aesthetics, our philosophy is to create subtle, natural-looking results that maintain your facial character whilst reducing the signs of ageing. We aim to turn back the clock, not erase your natural features. The best results leave you looking refreshed and rejuvenated – like a better version of yourself, not a different person entirely.

  • Both nasolabial lines and marionette lines last between 12-24 months, though individual results may vary.

Longevity depends on several factors including:

    • The specific product used: We use medium- and high-density fillers

    • Your metabolism and lifestyle: People with faster metabolisms may break down the filler more quickly; factors such as exercise frequency, stress levels, and overall health all play a role

    • The depth of the lines treated: Deeper, more established lines may require more product initially and could see the filler metabolise differently than treatment of moderate lines

    • Your age and skin condition: Younger patients with better skin quality and collagen production may experience longer-lasting results; sun damage and smoking can reduce longevity

  • Results are visible immediately, though optimal results appear after 14 days once any swelling has subsided and the product has fully settled into place.

  • This varies by individual. Typically, treating nasolabial lines requires 1-2 syringes, and marionette lines require 1-2 syringes. During your consultation, Dr Karwal will assess your needs and provide specific recommendations.

  • Most patients report minimal discomfort. The fillers we use contain lidocaine, and we can apply topical numbing cream beforehand. Most describe the sensation as a slight pinch or pressure. Any discomfort is brief and well-tolerated.

  • Common, temporary side effects include mild swelling, redness, bruising, and tenderness at injection sites. These typically resolve within a week.

  • Yes. Hyaluronic acid fillers can be dissolved using an enzyme called hyaluronidase if needed.

  • There's no specific age. Treatment is based on the presence and severity of lines rather than age. Some people develop these lines in their 30s, while others don't notice them until later. The best approach is preventative – treating lines when they first appear can prevent them from becoming deeper.

  • Absolutely. Many patients combine filler treatments with other procedures such as anti-wrinkle injections, skin boosters or dermal fillers in other facial areas for comprehensive facial rejuvenation.
