Is Tear Trough Filler the Right Treatment for You?

You get a full night's sleep, you drink plenty of water, and you've tried every eye cream the high street has to offer, yet every time you look in the mirror, tired, dark, hollow eyes stare back at you. If that sounds familiar, you're not alone. It's one of the most common frustrations we hear from patients at Karwal Aesthetics, and the good news is that there is a solution that actually works.

Tear trough filler is one of the most effective non-surgical treatments available for rejuvenating the under-eye area, but it isn't the right option for everyone. Understanding whether you're a good candidate is the key to getting results you'll love. In this blog, we'll explain how the treatment works, who it's best suited for, who may need an alternative approach, and what to expect if you decide to go ahead.

What Are Tear Troughs?

The tear trough is the groove that runs from the inner corner of the eye down towards the cheekbone, sitting between the lower eyelid and the upper cheek. As we age, the soft tissue and fat pads in this area naturally diminish, causing the skin to sink inward and create a visible hollow. Lifestyle factors such as stress, weight loss, poor sleep, and genetics can accelerate this process, making the hollowing appear earlier or become more pronounced.

When the tear trough becomes sunken, it casts a shadow beneath the eye that creates the appearance of dark circles. No amount of concealer, eye cream, or sleep will fix this, because the issue isn't at the surface of the skin. It's a structural loss of volume beneath it, and that's exactly what tear trough filler is designed to address.

How Does Tear Trough Filler Work?

Tear trough filler works by restoring the lost volume beneath the eye using a hyaluronic acid based dermal filler. By carefully placing a small amount of filler into the sunken tear trough, the depression is smoothed out, the dark shadowing is reduced, and the eyes instantly appear brighter, more refreshed, and more youthful.

At Karwal Aesthetics, we use Teosyal Redensity 2, which is the thinnest filler available on the market. Because the skin beneath the eyes is the thinnest on the entire face, using a lightweight, low density filler is essential to achieving smooth, natural results without puffiness or visible irregularities.

The filler is deposited beneath the skin using a cannula, a fine, blunt tipped instrument that allows Dr Karwal to place the product precisely and gently with minimal trauma to the tissue. A topical anaesthetic is applied beforehand to numb the area, though most patients describe the sensation as nothing more than a mild pulling or pressure. The procedure is extremely gentle, and the majority of patients experience only minimal swelling that clears within a couple of days. Occasionally, a small bruise may appear, but this typically resolves quickly.

Are You a Good Candidate?

Tear trough filler delivers its best results for patients who have visible hollowing or volume loss beneath the eyes that creates dark shadows and a tired appearance. If your main concern is that sunken, hollow look that makes you seem exhausted regardless of how well rested you are, there's a strong chance that tear trough filler could be transformative for you.

During your consultation at Karwal Aesthetics, Dr Karwal will carefully analyse your under-eye area to determine whether you are a suitable candidate. He will assess the type and severity of the hollowing, the quality and thickness of your skin, and whether the concern is related to volume loss, pigmentation, structural issues, or a combination of these.

Some patients also have a degree of volume loss in the cheek area, which can contribute to the tear trough appearing deeper than it actually is. In these cases, Dr Karwal may recommend cheek filler as an additional treatment to provide extra support to the under-eye area and create a more balanced, harmonious result.

Who May Not Be Suitable?

While tear trough filler is an excellent treatment for the right candidate, it is not appropriate for everyone, and a responsible practitioner will always be honest about this.

Patients with significant under-eye fat herniation or heavy bags may not be suitable for tear trough filler. Fat herniation occurs when the fat pads beneath the eye push forward, creating prominent, puffy bags. Adding filler to this area can actually make the problem worse by increasing the volume in a region that already has too much. For these patients, a surgical approach such as a lower blepharoplasty may be the more appropriate solution, and Dr Karwal will advise you openly if he believes this is the case.

Patients whose dark circles are primarily caused by hyperpigmentation rather than volume loss will not see a dramatic improvement from tear trough filler alone. Hyperpigmentation is a darkening of the skin itself, caused by excess melanin production, and it requires a different treatment approach. At Karwal Aesthetics, we recommend prescription grade topical treatments from the Obagi range to help regulate melanin production and brighten the under-eye skin. However, if you have both hyperpigmentation and hollowing, you can still benefit from tear trough filler to address the volume loss, combined with a targeted skincare plan for the pigmentation.

Patients with very thin, crepey skin or significant skin laxity may find that filler alone does not fully address their concerns. In these cases, Dr Karwal may suggest complementary treatments such as skin boosters, polynucleotides, or laser treatments to improve the skin quality alongside the filler for a more comprehensive result.

What Results Can You Expect?

The results of tear trough filler are visible immediately, with the full effect settling over the following two weeks as any minor swelling subsides. The transformation can be striking. Patients often tell us that they look like a completely different person, not because they look unnatural, but because they finally look like themselves again. Brighter, more awake, more refreshed, and more confident.

Results typically last between 12 and 24 months, depending on the individual and the amount of filler used. Many patients find that repeat treatments last even longer, and a simple maintenance appointment can keep the results looking fresh year after year.
