First-Time Anti-Wrinkle Treatment Guide

If you've been thinking about Botox but haven't quite taken the step, the unknown is usually what holds you back. What does it actually feel like? How long does it take? Will people be able to tell? These are the questions we hear most often from first-time patients — and they're all completely reasonable.

Here's an honest, straightforward walkthrough of what happens from the moment you book to the weeks after your treatment.

The Consultation

Everything starts with a conversation. During your consultation, Dr Arun Karwal will ask about your concerns, what's bothering you, and what kind of result you're hoping for. He'll assess your facial anatomy, look at how your muscles move when you make different expressions, and explain exactly which areas he'd recommend treating — and why.

This isn't a rushed five-minute chat before picking up a needle. Dr Karwal uses this time to understand your face properly, discuss what's realistic, and make sure you're comfortable with the plan before anything happens. If you decide you'd like to go ahead on the same day, you can. If you'd prefer to take some time to think about it, that's absolutely fine too.

The Treatment

The procedure itself is quicker than most people expect. Dr Karwal will cleanse the treatment area and use a very fine needle to place small amounts of botulinum toxin into the targeted muscles. Most patients describe the sensation as a brief, mild pinch — not pleasant, but nothing that requires numbing for the majority of people.

The whole thing typically takes around 15 to 20 minutes, depending on how many areas are being treated. There's no downtime. You can go straight back to work, run errands, carry on with your day.

Straight After

You might notice some slight redness or tiny bumps at the injection sites. These usually settle within an hour or so. Occasionally there's minor bruising, but it's not common and tends to be small enough to cover easily.

Dr Karwal will send you aftercare instructions via text, but the key points are simple: don't rub or press on the treated areas for the rest of the day, skip intense exercise for 24 hours, and avoid excessive heat (saunas, hot yoga, that sort of thing) for a day or two.

When You'll See Results

This is the part that requires a little patience. You won't walk out looking different. Anti-wrinkle injections take a few days to start working, and the full effect usually settles in around 10 to 14 days. The lines will gradually soften, your forehead will feel less heavy, and the overall effect is that you simply look more rested and relaxed.

If you're worried about looking "frozen" — this is where Dr Karwal's approach matters. The goal is always to reduce lines while keeping natural movement in your face. You should still be able to raise your eyebrows, frown, and express yourself normally. The result should be something only you really notice.

Your Two-Week Review

Dr Karwal schedules a follow-up at the two-week mark to check how everything has settled. This is your opportunity to discuss whether you're happy with the result, whether any areas need a small adjustment, and to ask any questions that have come up since your treatment.

Results typically last between three and four months for a first treatment, often longer with subsequent sessions as your muscles gradually become less overactive.
